A Simple Method for Fixation of Proximal Opening-Wedge Osteotomy of the First Metatarsal for Correction of Hallux Valgus.

چکیده
A simple, inexpensive technique for fixation of proximal opening-wedge osteotomy of the first metatarsal for correction of moderate or severe hallux valgus (HV) is described. After the opening-wedge osteotomy and bone grafting of the first metatarsal have been performed, 2 Kirschner wires are introduced for internal fixation and removed 8 weeks postoperatively. Twenty-three patients with symptomatic HV who had a proximal medial opening-wedge osteotomy of the first metatarsal in combination with a distal soft tissue procedure and bunionectomy were evaluated retrospectively. All osteotomies healed without complications and satisfaction was achieved in 22 patients. Hallux varus developed in 1 patient. Preoperatively, mean HV angle (HVA) was 41° (range, 35°-61°) and mean 1-2 intermetatarsal angle (IMA) was 19° (range, 16°-24°). Postoperatively, mean HVA was 14° (range, 10°-17°) and mean 1-2 IMA was 7° (range, 5°-9°). The mean decrease in the HVA was 27° (P<.001) and the mean decrease in the 1-2 IMA was 12° (P<.001). Surgical strategies: Ludloff first metatarsal osteotomy.
Moderate to severe hallux valgus deformities associated with a wide first intermetatarsal angle require a first metatarsal osteotomy for adequate correction. A variety of procedures have been introduced such as the crescentic, chevron, scarf, closing or opening wedge osteotomies of the first metatarsal base, and the Ludloff and Mau procedures.7,8,10,15,16,17,24,27 Recently the Ludloff osteotomy...
متن کاملFirst Metatarsal Proximal Opening Wedge Osteotomy for Correction of Hallux Valgus Deformity: Comparison of Straight versus Oblique Osteotomy
PURPOSE The aim of this study was to compare clinical and radiographic outcomes of proximal opening wedge osteotomy using a straight versus oblique osteotomy. MATERIALS AND METHODS We retrospectively reviewed 104 consecutive first metatarsal proximal opening wedge osteotomies performed in 95 patients with hallux valgus deformity. Twenty-six feet were treated using straight metatarsal osteotom...
متن کاملThe Proximal Opening Wedge Osteotomy for the Correction of Hallux Valgus Deformity
Many surgical procedures exist to correct metatarsus primus varus associated with hallux valgus deformity. Among these procedures are various first metatarsal osteotomies. Both distal and proximal osteotomies have been described to correct these deformities with the latter typically used for the more severe deformity. The goal of the operative procedure is correction of the intermetatarsal and ...
متن کاملStability of the First Metatarsal Base Wedge Osteotomy
INTRODUCTION Base wedge osteotomies are a wellestablished surgical indication for patients presenting with hallux valgus deformities. Proximal osteotomies are indicated for severe deformities in cases when the intermetatarsal angle is greater than 15 degrees (Landsman and Vogel, 1992). Numerous base wedge procedures have been identified to correct hallux valgus deformity, all requiring internal...
متن کاملKeller's arthroplasty with proximal metatarsal opening wedge osteotomy in treating severe Hallux valgus deformity--surgical techniques and case report.
The paper presents an operative technique proposed for treating severe Hallux Valgus deformity with arthrosis of the first metatarsal joint in elderly people. The procedure consists of the Keller's arthroplasty in combination with basal osteotomy of the first metatarsal bone, whereby the head of the metatarsal is displaced laterally and plantarward. The osteotomy is fixed with a reconstructive ...
